Whiplash After a Car Accident: What You Need to Know

Around 500,000 whiplash claims are made in the UK each year, making it one of the most common personal injury claims — and one of the most commonly misunderstood conditions. Here's what you need to know if you've been involved in a road traffic accident.

What is whiplash?

Whiplash is a soft tissue injury to the neck caused by a sudden acceleration-deceleration force — typically a rear-end collision. The rapid forward-backward motion causes the cervical spine structures (muscles, ligaments, discs, and joints) to be strained beyond their normal range.

Symptoms and when they appear

Whiplash symptoms often develop 12–24 hours after the accident, which is why many people leave the scene feeling fine only to experience significant pain the following day. Common symptoms include neck pain and stiffness, headaches, shoulder and upper back pain, dizziness, difficulty concentrating, and fatigue.

The importance of early physiotherapy

Early, active rehabilitation is the gold standard for whiplash treatment. Contrary to older advice, immobilisation in a collar is no longer recommended — evidence shows that early active movement leads to faster and more complete recovery. Physiotherapy can significantly reduce the risk of developing chronic whiplash associated disorder (WAD).

What to expect from physiotherapy treatment

Initial treatment focuses on pain management, restoring cervical range of movement, and addressing muscle spasm. As symptoms improve, specific strengthening exercises for the deep cervical flexors are introduced. Most uncomplicated whiplash resolves within 6–12 weeks with appropriate physiotherapy.
